Subject: [PATCH] Fix bill print summary rows colspan - align amounts to Amount
 column

- Change colspan from 3 to 4 (normal mode) and 4 to 5 (discount mode)
- Ensures Gross Total, Net Amount, Payment Recd., Balance amounts appear in Amount column
- Fixes blank cells in Rate column by properly spanning Description through Rate columns
