+## Incident Note: CBC Template Missing on Remote (`Lab_Admin/report_list`)
+
+### Symptom
+- On local, query returns CBC template:
+  - `SELECT mstRepoKey, Title FROM lab_repo WHERE Title LIKE '%CBC%';` -> 1 row
+- On remote, same query initially returned 0 rows.
+- Pathology template list also showed mixed/non-pathology items in some states.
+
+### Root Cause
+1. Seed data in `app/Database/Seeds/LabOemData/03_lab_repo.sql` contains:
+   - row with `mstRepoKey = 0` (`Manual`)
+   - row with `mstRepoKey = 1` (`COMPLETE BLOOD COUNT (CBC)`)
+2. During remote seeding, session SQL mode did not enforce `NO_AUTO_VALUE_ON_ZERO`.
+3. MySQL treated insert key `0` as auto-increment, effectively consuming key `1`.
+4. CBC row (`mstRepoKey=1`) then got skipped due to `INSERT IGNORE` key collision.
+5. Result: remote `lab_repo` missing CBC despite seed file being correct.
+
+### Permanent Code Fixes Applied
+
+1. Seeder SQL mode fix (critical)
+- File: `app/Database/Seeds/LabTemplateSeeder.php`
+- Change:
+  - Save old SQL mode: `SET @OLD_SQL_MODE = @@SESSION.sql_mode`
+  - Set session mode for seed: `SET SESSION SQL_MODE = "NO_AUTO_VALUE_ON_ZERO"`
+  - Restore old mode after seed.
+- Purpose: preserve explicit key `0`, prevent `0 -> 1` collision and CBC skip.
+
+2. One-shot remote repair command
+- File: `app/Commands/RefreshLabTemplates.php`
+- Command: `php spark oem:refresh-lab-templates`
+- Behavior:
+  - Truncates template tables only: `lab_repotests`, `lab_tests_option`, `lab_tests`, `lab_repo`, `lab_rgroups`, `radiology_ultrasound_template`
+  - Does **not** truncate `hc_items`
+  - Runs `LabTemplateSeeder`
+  - Verifies CBC exists post-refresh.
+
+3. Report list robustness (pathology list behavior)
+- File: `app/Controllers/Setting/Template.php`
+- `report_list()` adjusted to avoid hard dependency on `hc_items` existence while still applying safer filtering logic.
