Make quick-add modal larger (modal-lg) for better UX

IMPROVEMENT: Increased modal size for better readability

Changed modal-dialog from default to modal-lg
- Provides more space for SNOMED CT and ICD code fields
- Better readability for field labels and help text
- More comfortable data entry experience
- Consistent with other modals in the system
