aa7ed25a50 \Update documentation - {{DISCHARGE_SUMMARY}} now header only 
Updated DISCHARGE_PLACEHOLDER_HTML_REFERENCE.md to reflect that:

- {{DISCHARGE_SUMMARY}} now contains ONLY the status header
- Removed department/doctor/date info from example
- Added note about using separate tokens for additional info:
  - {{DEPARTMENT}}
  - {{DOCTOR_NAMES}}
  - {{ADMIT_DATE_ONLY}}
  - {{DISCHARGE_DATE_ONLY}}
  - {{ADMISSION_TIME}}
  - {{DISCHARGE_TIME}}

Example updated to show clean header only:

0b838fd6b2 Add comprehensive HTML reference for all discharge placeholders 
Created detailed documentation showing exact HTML structure for each discharge summary placeholder.

Document includes:

Section Placeholders:
- {{DISCHARGE_SUMMARY}} - Dynamic status-based header + department/doctor info
- {{PRESENTING_COMPLAINTS}} - Complaints list with remarks
- {{PAIN_MEASUREMENT_SCALE}} - Pain scale value
- {{GENERAL_EXAM_ADMISSION}} - Admission vitals and exam
- {{PERSONAL_HISTORY}} - Patient history flags (smoking, alcohol, etc.)
- {{DRUG_ALLERGY_ADR}} - Allergy and ADR information
- {{CO_MORBIDITIES}} - Comorbid conditions
- {{CLINICAL_INVESTIGATION_REPORTS}} - Lab reports table/list
- {{FINAL_DIAGNOSIS}} - Final diagnosis list
- {{COURSE_IN_HOSPITAL}} - Hospital course narrative
- {{EXAMINATION_ON_DISCHARGE}} - Discharge examination findings

- Shows exact HTML structure with styling
- Includes CSS inline styles used
- Shows alternate formats when applicable
- Indicates when placeholder is empty/not shown
- Shows color codes (#475569 for remarks)

Style Guidelines:
- Heading margins: 16px top, 8px bottom
- List indentation: 20px left
- Remark color: #475569 (gray)
- Table borders: collapse style
- All user input escaped with esc()
